I am considered to be an out-of-network provider for all insurance companies. Most insurance companies do provide out-of-network coverage and this amount will vary from policy to policy so please read your mental health/behavioral health coverage section carefully to determine your benefits. Full payments are made at the time of service and the appropriate paperwork will be provided to you to submit along with your claim forms to your insurance provider for the reimbursement that you are due.
Payments can be made by credit card, including FSA and HSA cards, cash, or check made payable to Anita M. Schimizzi, Ph.D.
